How do I FICA with World Sports Betting

In order to FICA with World Sports Betting, we require the following documentation

Copy of ID
Utility Bill ie Proof of Address (Not older than 3 months) The following "Proof of Address" documents may be used:
- Utility bill less than three months old
- Bank statements less than three months old
- Rates invoice from your municipality less than 3 months old
- Home loan/bond statement less than 3 months old
- A Certified Affidavit
